Call Procedure
1.
Bring a copy of current orders and report to 4th floor Managed Care
(x1049) at the
Mountain
entrance with ID card for inprocessing.
2.
For social security numbers that end in an even number:
Proceed to Family Practice (room 1D350, x3300) for care
Monday - Friday from
7:30-8:00 AM
3.
For social security numbers that end in an odd number:
Proceed to the Adult Medicine
Clinic (room 3H501, x6641) for care
Monday - Friday from
7:15-8:15 AM

Examination Information
If you are a TPU soldier
assigned to a unit then your unit administrator goes onto the MEDPROS website
and requests the services you need via the AVS (Automatic Voucher System).
This voucher is then forwarded via the system to your RSC or other approval
authority. Once approved it goes to the FEDS-HEAL program office to have a
provider assigned. The completed voucher is then emailed back via the AVS
to your unit administrator to be printed and given to you.
If you are an IRR or IMA
reservist then call US Army Human Resources Command (HRC) at 1-800-452-0201.
The customer service representative will put you in contact with the team
in charge of your particular MOS/AOC and they will initiate the procedure.

Active Duty
Medical Extension and Line of Duty
Active Duty Medical Extension (ADME)
- Reserve Component (RC) personnel may be retained on active duty when an injury
or illness occurs in the line of duty while on active duty orders or in an
inactive duty training (IDT) status.

Line of Duty (LOD) -The LOD
investigation is used to determine and document eligibility for medical care.
If a reservist is injured while training at Tripler Army Medical Center (TAMC),
report to Sick Call or the ER and contact Patient Affairs to complete DA Form
2173, statement of Medical Examination. 9th RSC personnel will take DA
Form 2173 to 9th RSC Personnel Office for processing. TAMC IMAs / IRRs
will take DA Form 2173 to Schofield Barracks for processing. Other unit
and individual reservists will take
DA
Form 2173 to their RC unit for processing.
