The TAMC Form 108 is also used in referring soldiers for evaluations required by regulation: Chapters 13, 14 (sec III), or 15. A mental status evaluation is also required when a soldier being processed for discharge under Chapter 10 requests a medical examination.

When making a command directed evaluation request, call Division Mental Health at 808-433-8600, Community Mental Health at 808-433-8580, Department of Psychiatry at 808-433-2737 or Department of Psychology at 808-433-1498. You must call the clinic to schedule the appointment ahead of time.

1. EMERGENT COMMAND DIRECTED MENTAL HEALTH EVALUATION
Commanders must refer soldiers for a mental-health evaluation who show signs of endangering themselves or others.

A commander shall refer a soldier for an emergency mental health evaluation as soon as is practical whenever a soldier indicates an intent to cause serious injury to himself or others, and the commander believes that the soldier may be suffering from a mental disorder.

Prior to transporting a soldier for an emergency evaluation, or shortly thereafter if the time and nature of the emergency does not permit, the commanding officer shall consult directly with a mental health professional. The purpose of this consult is to communicate the observations and circumstances which led the commander to believe that the soldier’s behavior constituted an emergency. The commander will then forward the TAMC Form 108 * to the mental health professional, documenting the information discussed.
2. NON-EMERGENT COMMAND DIRECTED MENTAL HEALTH EVALUTION:

Contact Warrior Behavioral Health 808-433-8600, Department of Psychiatry at 808-433-2737 or Department of Psychology at 808-433-1498 and speak directly with a mental health professional to request a command-directed mental health evaluation. Signs of mental illness can include changes in behavior, mood, or thinking that interfere with normal functioning. The mental health professional will clarify the request, urgency of the referral, and schedule an appointment.

Complete and sign a
Counsel the soldier and insure that the soldier is provided written notice of the referral:
(1) A brief, factual description of the soldier's behavior and verbal expressions that indicate a mental-health evaluation is necessary.
(2) Names of mental health professionals the commander has consulted before making the referral. If consultation is not possible, commanders must include the reasons in the notice.
