• For those new staff members and residents being assigned to the Family Medicine Clinic and the Family Medicine Residency Program please visit the Family Medicine Residency Program page for required reading by all staff and residents.

• For those individuals seeking to become a credentialed provider, please contact our credentialing office at (808) 433-2460 to make sure that your credentialing paperwork has been received and if not, find out what is needed to start your credentialing process and fax it to them at (808) 433-1558.

• Please complete the Newcomer Information form to better acquaint us with you, and assign you a sponsor and assist you in your move to Hawai'i.

  • Long Term Accommodations: Hawaii Real Estate Central provides on-line access to rental properties and other housing information.
