HOW TO APPLY FOR A CLERKSHIP
Uniformed Services University of Health Sciences (USUHS) Students

Uniformed Services University of the Health Sciences (USUHS) students do not need to apply for clerkships using the Tripler Army Medical Center (TAMC) online application form.  Third year rotations are pre-arranged by USUHS. 

Fourth year USUHS students are required to fill out USUHS Form 1304 and must FAX this form to the TAMC Medical Education Office at (808) 433-1559 if they want to schedule a rotation.  If you are requesting more than one rotation, fill out a separate form for each rotation.  If you cancel a rotation, complete USUHS Form 1304, check the appropriate box, and then fax it to the TAMC Clerkship Coordinator.  If you need to change your rotation or dates of training, send a cancellation, plus a new USUHS Form 1304 with the changes.  Inform the TAMC Clerkship Coordinator of any changes by emailing TAMCStudentClerk@amedd.army.mil.

Always discuss your schedule with the appropriate person at USUHS for approval first to eliminate any last minute changes or cancellations.  

Remember that our medical center is holding a slot specifically for you and it is your responsibility to notify the TAMC Clerkship Coordinator immediately of any changes in your schedule or your inability to participate in a scheduled rotation.  If the Medical Education Office is notified early of a cancellation, the rotation can be offered to another student.
