TRIPLER ARMY MEDICAL CENTER (TAMC) CHECKLIST
WHAT I NEED TO BRING TO TAMC FOR INPROCESSING
 

Dear 2007-2008 first year residents,

Thank you for visiting our website. We are in the process of updating and redesigning our site to better serve you, and we ask for your patience throughout this process.

We will contact all first year residents selected to start training at Tripler Army Medical Center in the summer of 2007 via email shortly after the New Year to provide them with further information about reporting and inprocessing at our institution.

You should start now to locate and consolidate all of the documents listed below.  These documents are needed to create your personnel record which will stay with you throughout your Army career.  Failure to bring necessary items will cause delays in processing your pay and entitlements.  Bring original documents.  Do not ship documents in your hold baggage or household goods!  It is also imperative that you bring all documents pertaining to prior military service.  Personnel records will be created at the Intern Reception Center (centralized inprocessing) which will be held on a date to be announced soon.


Uniforms:
 
Class B for centralized inprocessing at the Intern Reception Center.
Class B for the commander’s welcome briefing.
Class B for intern composite picture taking (face shots) - Medical Corps interns only.
Class A when you take your official full-length photo.
Class B or Battle Dress Uniform (BDU) for other scheduled activities in June.

Documents Needed by the Military Personnel Office:
 
(  )  DD 214, NGB 22, enlistment contracts, and / or separation orders.  (To document all periods of prior service, if applicable).
 
(  )  Letter from medical school / other training school stating start date, end date, and type of degree granted (you will need 2 letters - one for Military Personnel and a second letter for the TAMC Medical Education Office - for your credentials file).
 
(  )  Bring original medical / other school diploma - official translation document required if diploma written in Latin - recommend you not frame diploma until after inprocessing.
 
(  )  One (1) set of raised seal medical / other school transcripts.
 
(  )  2LT Oath of Commissioning (DA Form 71).
 
(  )  CPT Oath of Commissioning (DA Form 71).
 
(  )  Letter of appointment as a commissioned officer.
 
(  )  Order to active duty (PCS and original order to active duty for USUHS).
 
(  )  Officer Basic Course completion certificate (if applicable).
 
(  )  Advanced Cardiac Life Support Training Certification, if qualified.
 
(  )  All other military training certificates.
 
(  )  Birth certificates for all family members (original and 2 copies / original will be returned).
 
(  )  Marriage certificate (original and 2 copies / original will be returned).
 
(  )  Proof of citizenship (certified birth certificate / passport).
 
(  )  Loan repayment waivers.
 
(  )  Copy of contracts entered for HPSP, USUHS, ROTC, other educational programs.

Documents Needed for Finance / Personnel Administrative Center (PAC):
 
(  )  DA Form 31 (Authorization for Leave) (Original and all copies) if applicable.
 
(  )  Airline tickets (3 copies).
 
(  )  Orders sending you to Hawaii with all amendments (10 copies).
 
(  )  DD Form 788 (vehicle shipment) (2 copies).
 
(  )  SF 1199 (Authorization for deposit) if federal recurring payments).
 
(  )  Travel or other reimbursable receipts (3 copies).
 
(  )  Uniform allowance (initial and active).
 
(  )  Advance pay paperwork (2 copies) if applicable.
 
(  )  Lease and rental agreements, if applicable (include all related documents).
 
(  )  Vehicle registration, title, and insurance.
 
(  )  If married prior to arriving in Hawaii, dependent travel must be authorized on the officer’s order (prior to departure).

Other Documents Needed for In-processing:
 
(  )  Medical records.
 
(  )  Dental records.
 
(  )  Immunization records.
